Selecting a Statement of Organization Form

You can access the Registration page to select a Statement of Organization form to complete and submit to the Secretary of State's Office for approval.

Access the page from the left navigation menu...

Click the committee registration link in the left navigation menu.

 

The Registration page is displayed.

Access the page from the menu bar...

Click the registration tab in the menu bar.

 

The Registration page is displayed.

 

To select a Statement of Organization form:

 

Note: Required fields are indicated by an asterisk * to the right of the label and must be completed.

Complete a form for a Candidate...

  1. Click the candidate (cfa-1) link.

 

The Statement of Organization for Candidate (CFA-1) page is displayed.

 

  1. Click the next button. Tip icon To close the page without submitting the form, click the return button.

 

  1. Perform the steps for Completing a CFA-1 Form.

Complete a form for a Political Action Committee...

  1. Click the PAC (cfa-2) link.

 

The Statement of Organization for PAC or Legislative Caucus (CFA-2) page is displayed.

 

  1. Click the next button. Tip icon To close the page without submitting the form, click the return button.

 

  1. Perform the steps for Completing a CFA-2 Form.

Complete a form for a Legislative Caucus...

  1. Click the legislative caucus (cfa-2) link.

 

The Statement of Organization for PAC or Legislative Caucus (CFA-2) page is displayed.

 

  1. Click the next button. Tip icon To close the page without submitting the form, click the return button.

Complete a form for a Regular Party Committee...

  1. Click the regular party committee (cfa-3) link.

 

The Statement of Organization for Regular Party Committee (CFA-3) page is displayed.

To check the status of a form you submitted online:

  1. In the Check on Form Submission Status section of the Registration page, click the click here link.

 

The Committee Registration Status Check page is displayed.
